Performance Questionnaire

 There are many hours of preparation that are invested into bringing an artist or minister to a venue for an event. 
This questionnaire will assist us in properly beginning the booking process. 

 Please note that completing this questionnaire serves as an invitation only, not as a formal commitment or confirmation to appear. 
Provide as much information as possible.  It will assist us in making the best decision possible as we endeavor to answer your request.

 The Princess Productions Company, Inc. does require a minimum honorarium of $2,500.00 for appearances
by its artists. Travel, hotel fees, and ground transportation are the responsibility of the group/organization booking the artist.
